To apply for, replace, or renew a New Jersey disability parking placard or license plates, here's what you need:
New Permanent Placard or License Plates
- Form SP-41, completed and signed by a licensed medical practitioner
- Original prescription for your condition (or letter on letterhead if practitioner cannot write prescriptions)
- For license plates: photocopy of current vehicle registration
- If no driver's license or ID card: documents meeting 6 Points of ID verification requirements
New Temporary Placard
- Form SP-68, completed and signed by a licensed medical practitioner
Veteran Exemption
Veterans with disabilities or Purple Heart recipients may qualify for a special parking meter exemption placard. Submit:
- Veterans with disabilities: DD-214 or DD-215 AND Award of Disability letter from the Veterans Administration
- Purple Heart recipients: DD-214 or DD-215 showing Purple Heart status OR citation awarding the Purple Heart OR General Order for the Purple Heart
Online Doctor's Certification
If applying through a telehealth service like ParkingMD, you can get your evaluation done completely online. You'll receive:
- A completed Form SP-41 or SP-68 signed by your medical practitioner with required prescription or letter, ready for New Jersey MVC submission
Lost or Stolen Placard Replacement
If your placard is lost or stolen, you'll need to provide:
- Form SP-41 (medical certification not required if current permit is still valid)
- Check the "Replacement" box
- Original Person with a Disability Identification Card
- If stolen: copy of police report; if lost: notarized statement
