Disability Parking in Oklahoma:
Everything You Need to Know

• Oklahoma Disability Parking Laws

  • Applicant must be unable to walk 200 feet without rest or assistance (brace, cane, wheelchair, another person, etc.).
  • Qualifies if using portable oxygen, has FEV ≤ 1 liter, oxygen tension < 60 mmHg, or Class III/IV heart condition.
  • Also covers those with arthritis, neurological, orthopedic limits, pregnancy complications, blindness, or limb loss.
  • Must be certified by a licensed physician, PA, APRN, optometrist, podiatrist, or chiropractor (based on condition).
  • Temporary placard: valid up to 6 months, requires doctor-specified end date, renewable with a new form.
  • Permanent placard: valid 5 years, no fee, renewable with a new application after expiration.
  • Lifetime placards are no longer issued.
  • Apply using Form DPS 302-DC from Service Oklahoma; medical certification must be within 60 days of submission.
  • Placard must hang from the rearview mirror when parked and is person-specific, not vehicle-specific.
  • Allows parking in designated accessible spaces only — not in fire lanes or loading zones.
  • Disability plates may be issued if you hold a valid 5-year placard.
  • Misuse or false application is a misdemeanor, punishable by up to $500 penalty.

• Documents You’ll Need

  Temporary (6-Month) or Permanent (5-Year) Placard

  • Submit Form 302DC signed by a licensed medical professional.
  • Provide your Oklahoma driver’s license or state ID number.

  Lost or Stolen Placard Replacement

  • Complete Form 302DC for replacement.
  • Include your original placard number (if available) and proof it was valid when lost or damaged.

  Placard Renewal

  • Submit Form 302DC with a new medical certification.
  • Provide a valid Oklahoma driver’s license or state ID (or physician exemption).
  • Include your current placard number if available.

  Online Doctor’s Certification

  • Through telehealth (e.g., ParkingMD), get a signed Form OK DPS 302DC 002 by email within 24–48 hours for DMV submission.
  • Only one replacement per placard term is allowed.
  • Doctor must confirm eligibility for the disability permit.

• Renewal & Replacement

  • Check your placard’s expiration date (temporary: 6 months, permanent: up to 5 years).
  • Have a licensed healthcare professional certify your condition for renewal.
  • Fill out Form 302DC and mark “Renewal.”
  • Include a new physician certification dated within 60 days (Section 2).
  • Add your driver’s license or state ID number.
  • Submit the form within 60 days of the doctor’s signature.
  • You can submit online, by mail, or in person at a Service Oklahoma office.
  • Receive your renewed disability placard once approved.

  Disability Parking Permit Replacement Process

  • Complete Form 302DC (Section 1 only) and mark “Replacement (Lost/Stolen/Destroyed).”
  • Provide your original placard number (if known) and ID number.
  • Submit online, by mail, or in person through Service Oklahoma.
  • A new placard will be issued with the same expiration date as the original.
  • Only one replacement is allowed for each 5-year placard.
