• Oregon Disability Parking Laws

  New Permanent or Temporary Permit

  To apply for a new disability parking permit in Oregon, you'll need:

  • Valid Oregon Driver License or Identification Card
  • Form 735-265, with Certificate of Disability section completed and signed by licensed physician

  Veteran Exemption

  Wounded Warriors with 50% or greater service-connected disability may qualify for an Oregon Wounded Warrior permit. In addition to Form 735-265, submit:

  • Letter from Veterans' Administration indicating 50% or greater service-connected disability
  • Document reflecting discharge status other than dishonorable
